I perfer to take a premade module and change it. I have gone as far as completly changing monsters and story lines. Some areas I plan one thing, and then I just end up ad libbing the entire thing and it ends up going the opposite direction than it was supposed to. That is where my good stories come from. Like killing the party rogue with an awakened assasin mouse instead of just an assasin.
