I suggested such a short time frame because I want the HBO series to have a nice long run, but if it does then there's a risk the show will outpace the books, creating one or more problems like what we see in some animes.

is that because you don't think TV would do the high magic aspects of the story justice, or something else?

It's because I think Feast for Crows was ass, and Storm of Swords is the high point of th series. I wouldn't want them to waste the time and money on filming it. I am willing to give up seeing certain parts of Dance of Dragons (that I really, reaaaaallly want to see) in order to stop at the high point of the series.

I was thinking about that and it occurred to me that a TV audience wouldn't put up with a whole season of not getting their Tyrian fix, and the Martin himself described book 4 as only half of what he wanted for that installment; I suspect that there would be some structural reworking between the book and series at that time which I'm hoping will include separating the wheat from the chaff.

I'm pretty sure the answer is none. I remember trolling the reddit boards and a few of the forums right after the book came out and I don't think anyone supported that theory. The closest I've seen is that he didn't actually die, but was healed by Mellisandre as he was stabbed

I'm convinced he'll be back. Either he didn't die, or else he'll be resurrected as part of the slowest prophecy reveal in the history of literature.

He might be totally dead, but then, so was Katherine Tully/Stark and she was revived by red priests of R'hllor. It might be that Melisandre foresaw this and is prepared for such an eventuality.
