I continue to stand by wielding the entire mattress. The spring-shiv is entirely too normal and reasonable. Full-mattress is appropriately weird.

Craft a Spring Shiv.
 Yes a set of armor like this needs a weapon to accompany it! You walk over to the used mattress and pull out a loose spring.  Unfortunately you can't seem to get the piece of wire to straighten. Even if you could, you doubt you could classify it as a "shiv". It's just a bit of wire. Well it's better than nothing for now. You decide to turn your attention to the mysterious boxes that have been hanging around. You should probably do something about them. You don't want to have to go back to the hospital to get this hallucinations sorted out. You doubt Doctor Cyclopsopolis want's to see you so soon. What do you do?
