It actually bugged me looking at that picture that I couldn't figure out which breast was fake. The middle one would be the obvious choice, but then the other two would both be unnaturally far apart. Looking at the other two they seem far too off center, so I couldn't decide.
Then I actually read the linked article. Its all three. Her real ones are presumably wrapped.

I believe it's a mold that is cut to fit over her real ones, like a weird/awesome sort of bra. Then makeup is applied to match her flesh tone. Apparently it takes six hours to set everything up.
