Some people would think that a nameless npc must be pretty unimportant, but I can have put a lot of thought in without having thought to give them a name.

I have some weakness as a GM but but coming up with names on the fly has never been one.

Music, and lots of it. I find I often can't focus as a GM without music to help create the right mood.
I have a specific chair in the gaming room (appropriate since it's my house and it's my really comfy leather office chair I have sitting at my PC).
I'm not sure if it's my idiosyncracy or not, but ever since we moved in together, my wife has always sat just to my right at our games.

I don't always include little girls in my campaigns, but when I do, they're scary, and most likely evil as well.

I always, always have a guard in some town named Gerard. Dating back to my first campaign, where a half-dragon PC failed to intimidate a guard named Gerard. Ever since Gusty Guard Gerard has made an appearance in the game.

At one point, I started standing when I was GMing partly because it was just easier to see over the DM screen but also because I had too much energy to sit still. For a while, I kept my notes on a clip board and wandered around the players while I was running the game.
