I don't, but that reminded me of another one. I attach theme songs to campaigns that I listen to while ramping up to run for the day.

This, so very much this. Nowadays I use random generators and filter out the stupid ones or use a series which I know my friends haven't read (though I have been caught out now we invited a PC who has read a song of ice and fire to an existing campaign).

I also have a tell, I tend to smile when the PC are playing into my hands or doing things that help my NPCs. This tends to come at the expense of my players most times so now when they see me smile they start to proceed with caution.

Also I tend to sit in a big armchair when I GM, all the better to look villianous in ;)

I have specific mood music for different campaigns. My first D&D world used the Academic Festival Overture by Brahms. Currently, it's the soundtrack to Conan. Modern music services like Pandora allow me to easily add without redundancy, which is nice.

Other idiosyncrasies. I have to sit on the short end of the table. I can't have players to my immediate right or left.

My NPCs always speak in character and in a unique voice. Accents do not need to make regional sense. My fantasy city might have a south London street punk voice for one npc and a southern drawl for another. The important thing is to paint a quick audio portrait of the character.

The first group I gamed with, whomever was GMing would be wearing fingerless driving gloves. No, I never did understand why.

I also stand while I gm. Unless the pc's are talking to someone who is sitting.

Though I often never get to eat any food that is brought to the session since I gm at the kitchen island while the players are at the table and I'm usually right in the game and forget to grab anything.

Coming up with names isn't so hard when you look at how people were named back in Medieval times. It was usually some name from a small handful of common ones and then some descriptive term. So you get a lot of Smith, Fletcher, Little, Small, Black, White, etc.

Some settings have some names already in place, particularly the modern setting. I'll usually do my research into those names to determine cultural heritage. I can then expand on that.

Sometimes I'll do very subtle things with names. For example: A man might be called Chun, obviously oriental. But if he's called Chung, or Jian or one of many other phonetic equivalents he might be from China or Vietnamese or even Korean. But he might put his family name (Lee) in front or behind his name. So Chun Lee might be an American descendent from a Chinese family with cultural heritage while John Lee (assuming oriental and not a southerner) might be the same but the family has suffered from racial oppression. Lee Chung would likely be Korean and places family ahead of himself. I tend to not have too many people with similar sounding names or even names with the same starting sound since it confuses players. But for a one off comedy of errors game at a convention, infiltrating the Li Chang gang where everyone's name was almost phonetically identical, I think that would be hilarious.
